Bureau of Vital Records and Health Statistics Moves Location

The New Mexico Department of Health Bureau of Vital Records and Health Statistics is moving to a new location, 2554 Camino Entrada, Santa Fe, NM 87507, and will be open to the public on February 8th, 2021. Vital Records will continue to offer services at its current location (1105 South St. Francis Dr.) through February 5th. 

Hours of operation will remain the same: Monday – Friday; 8:00 am to 4:00 pm. Customers who request in-person services are required to follow COVID-safe practices by wearing a mask and staying six feet apart. To call the bureau directly, dial (866)-534-0051 or (505) 827-0121. 

During this transition, Vital Records will continue to provide customer service and fulfill requests from the public. The bureau offers a variety of services, many of which are available online through VitalCheck, The public is encouraged to request services online for safety and security.  

Vital Records services include:

• Register birth and death certificates for the State of New Mexico
• Issue certified copies of filed Birth Certificates and Death Certificates
• Process Acknowledgement of Paternity requests
• Prepare data files for public health surveillance
• Publish health statistics related to birth and death data
