Rules

Forms

Consent

This form should be used by the parents of children applying for EMSFR or EMT-B licensure.

Initial

This form should be used for initial applicants.

Renewal

This form should be used for renewal applicants.

Re-Entry

This form should be used for re-entry applicants.

Reciprocity

This form should be used for reciprocity applicants.

Background Screening

The EMSB has joined the majority of states in the country by requiring all applicants for licensure, and current license holders, to submit to a state and nationwide criminal background check. Please see the following publications for more information.
