Patients & Caregivers
In 2021, the New Mexico Department of Health Medical Cannabis Program introduced an Online Patient Portal (The New Mexico Department of Health Medical Cannabis Program Online Portal).
The Online Patient Portal provides qualified patients and their designated caregivers with 24-hour access to check the status of their application, check their available units, review their sales history, and access their electronic card. In addition, when working with an authorized medical provider, they can complete and submit applications electronically.

Applications
Electronic Applications (processed within 5 business days of receipt)
When working with an authorized medical provider, you can complete and submit your applications electronically. Applications submitted through the Online Patient Portal are typically processed within 5 business days; you can access your electronic card upon approval (see instructions).
Paper Applications
NOTE: The program will no longer accept Paper Applications after April 1, 2023.

Information Change Application
If you have had a legal name change, visit the Online Patient Portal to complete an Information Change Application.
If you have lost your physical card, you can access your electronic card through the Online Patient Portal (see instructions).
Caregivers
You must be designated as a Caregiver by:
- A new Patient enrolling in the New Mexico Department of Health, Medical Cannabis Program.
- For a new or recertifying Patient, the Caregiver will be added to Patient application, so that the Medical Provider and Patient to approve.
- An existing Patient of the New Mexico Department of Health, Medical Cannabis Program.
- For an existing Patient adding a Caregiver, you will need to visit the Patients Medical Provider to start a Caregiver application. Once the Patient approves you as their Caregiver, you will receive an email notification to complete the application.
