Who can help me in the event of a serious health emergency?
We have several telephone lines available for different types of emergencies. Simply call the appropriate number from our Emergency Hotlines page and follow the verbal instructions.
Where do I obtain an official copy of shot records?
Your primary care provider should have access to the State's immunization registry and can print a copy of your immunization record to give to you. Parents can get their children's shot records from the primary care provider / pediatrician. If your primary care provider does not have access to the State's immunization registry, then you can go to your local Public Health Office with photo ID and request a copy from that office.
Where do I obtain an official copy of a birth or death certificate?
Head on over to the Vital Records section of our website for instructions on how to submit a records request. You will find all the information and required forms you need there.
We also have a Birth & Death Certificate Requests Hotline available. Simply call 1-866-534-0051 during normal business hours and one of our representatives will be happy to assist you.
How do I register a formal complaint against a healthcare provider?

Can I submit my web resource or link for inclusion on your site?
Unfortunately we cannot add links on our website to private businesses or organizations. While the resource you have to offer may be valuable to the public, we are unable to link at the risk of expressing preference or endorsing a particular commercial product or service.
Where can I find job postings for the Department of Health?
All of the State Personnel Office Job Opportunities with the New Mexico Department of Health (NMDOH) are made available on the State Personnel Office (SPO) website. Their website always contains the most up-to-date information and is the entry point to find and apply for all state jobs.
