  • NMAC 7.27.12 - Certification of S-T Segment Elevation Myocardial Infraction (STEMI) Receiving & Referring Centers
  • NMAC 7.27.13 - Emergency Medical Services Certification of Stroke Centers

Consent

This form should be used by the parents of children applying for EMSFR (need to be 16 yrs. old) or EMT-B licensure (need to be 17 yrs. old). Anyone 18 yrs. old does not need this form.
