Where do I obtain an official copy of shot records?

Your primary care provider should have access to the State's immunization registry and can print a copy of your immunization record to give to you. Parents can get their children's shot records from the primary care provider/pediatrician. If your primary care provider does not have access to the State's immunization registry, then you can go to your local Public Health Office with photo ID and request a copy from that office.
