All petitions must be received by the Medical Cannabis Program by Wednesday, March 21, 2012.
Copies of the agenda can be obtained 24 hours prior to the meeting.
Petition Requirements and Updates
For any individual wanting to present a petition to the Medical Advisory Board for consideration of a condition or disease, please review the requirements (New Mexico Department of Health Medical Cannabis Program – Petition Requirements) and note the date for submission. Petitions not received by March 21, 2012 will not be heard at the April 18, 2012 meeting. Petitions may be hand delivered or mailed to: Medical Cannabis Program, 1190 St. Francis Drive, S4250, Santa Fe, New Mexico 87502.
Application Forms
Old versions of application forms will no longer be accepted by the Medical Cannabis Program. By using the most up-to-date application forms and filling them out completely, you will help us to ensure that applications are processed as quickly as possible. Please go to Application Forms to download the new form packet. For patients re-enrolling in the program, the Patient Enrollment and Re-enrollment forms are now combined in one form which is available at Application Forms.
Producer Information
Contact information for licensed non-profit producers is only available to actively enrolled patients. Once a patient is enrolled in the program, a list of licensed non-profit producers is sent with the program enrollment card. This information is confidential and is not released to anyone other than enrolled patients. The Department is not currently accepting applications from non-profit businesses interested in producing and distributing medical cannabis for patients in New Mexico.
